An AI hallucination is a statement produced by an AI that sounds plausible but rests on no reliable source: an invented figure, date or quotation.

Language models produce the most likely text, not verified text. When information is missing, they can fill the gap convincingly: that filling-in is what we call a hallucination.
It cannot be eliminated entirely, but it can be limited: chosen sources, an instruction not to invent anything, an explicit request to flag gaps, and a review of sensitive statements.
